# Short Code Terms Template

## To get approved for a Dedicated Short Code (5-6 digit #), you will need to add a Short Code messaging program disclosure to your terms.

### Short Code Terms Template

If you are applying for a Short Code with Mobiz, you will need to add a Short Code specific section to your terms and conditions/privacy that are publicly accessible in the footer of the home page of your website.

**Below the line, is a template that you can use the following merge fields to find/replace with your information.**

- \[Company\_Name\]
- \[Short\_Code\]
- \[Support\_Email\]

**\*\* Important note** – if you do not have a phone number capture publicly accessible on the homepage of the website, please change the first line text accordingly.

*Examples:*

- Send START to \[Short\_Code\] or enter your phone number in the \[Company\_name\] account portal to get text alerts.
- Send START to \[Short\_Code\] to get text alerts.

**\[Company\_name\]** **SMS Alerts**

Send START to \[Short\_Code\] or enter your phone number on the \[Company\_Name\] website to get text alerts. You may receive a welcome message and subsequent account update messages.

\[Company\_Name\] does not charge for receiving text message alerts. Message frequency varies by account. Message and data rates may apply. Check your text messaging plan with your carrier. For help, text HELP to \[Short\_Code\] or email \[Support\_Email\]. To stop receiving text messages at any time, text STOP to \[Short\_Code\]. Supported carriers include: AT&T, Verizon, MetroPCS, Sprint, Boost, T-Mobile, Alltel, Virgin Mobile, U.S. Cellular. Carriers are not liable for delayed or undelivered messages.

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes.

All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
